Spellcasting. Qazalek is a 10th-level spellcaster. His spellcasting ability is Intelligence (save DC 18). Qazalek has the following wizard spells prepared:

  • Cantrips (at will): blade ward, mage hand, mind sliver, minor illusion
  • 1st level (4 slots): absorb elements, cause fear, shield, silvery barbs
  • 2nd level (3 slots): blur, invisibility, mirror image, misty step
  • 3rd level (3 slots): counterspell, dispel magic, fly
  • 4th level (3 slots): banishment, dimension door
  • 5th level (2 slots): hold monster, wall of force
  Spellcasting (Psionics). Qazalek casts one of the following spells, requiring no spell components and using Intelligence as the spellcasting ability (spell save DC 18):
  • At Will: detect thoughts, levitate
  • 1/Day each: dominate monster, feeblemind, mass suggestion, plane shift (self only), project image, scrying, telekinesis


Special Equipment. Qazalek is attuned to a Fractured Philosopher's Stone. While attune to this item, he benefits tom the Immortality and Rejuvenation features detailed below.   Creature Sense. Qazalek is aware of the presence of creatures within 2 miles of him that have an Intelligence score of 4 or higher. He knows the distance and direction to each creature, as well as each creature's intelligence score, but can't sense anything else about it. A creature protected by a mind blank spell, a nondetection spell, or similar magic can't be perceived in this manner.   Immortality (Philosopher's Stone). Qazalek does not age and is immune to any effect that would cause him to age.   Legendary Resistance (3/Day). If Qazalek fails a saving throw, he can choose to succeed instead.   Magic Resistance. Qazalek has advantage on saving throws against spells and other magical effects.   Psionic Hub. If an elder brain establishes a psychic link with Qazalek, the elder brain can form a psychic link with any other creature Qazalek can detect using its Creature Sense. Any such link ends if the creature falls outside the telepathy ranges of both Qazalek and the elder brain. Qazalek can maintain his psychic link with the elder brain regardless of the distance between them, so long as they are both on the same plane of existence. If Qazalek is more than 5 miles away from the elder brain, it can end the psychic link at any time (no action required).   Rejuvenation (Philosopher's Stone). If Qazalek dies, he gains a new body after 1D10 days, regaining all his hit points and becoming active again. The new body appears within 5 feet of a random fragment of his Philosopher's Stone.

Actions

Tentacles. Melee Weapon Attack: +10 to hit, reach 10 ft., one creature. Hit: 4D10+5 psychic damage. If the target is Large or smaller, it is grappled (escape DC 15) and must succeed on a DC 18 Intelligence saving throw or be stunned until this grapple ends.   Extract Brain. Melee Weapon Attack: +10 to hit, reach 5 ft., one incapacitated Humanoid grappled by Qazalek. Hit: 10D10 piercing damage. If this damage reduces the target to 0 hit points, Qazalek kills the target by extracting and devouring its brain.   Mind Blast (Recharge 5–6). Qazalek magically emits psychic energy in a 60-foot cone. Each creature in that area must succeed on a DC 18 Intelligence saving throw or take 4D12+5 psychic damage and be stunned for 1 minute. A target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Legendary Actions

Qazalek can take 5 legendary actions, choosing from the options below. Only one legendary action can be used at a time and only at the end of another creature's turn. Qazalek regains spent legendary actions at the start of its turn.

  • Cast Cantrip (Costs 1 Action). Qazalek casts a cantrip.
  • Cast 1st-level Spell (Costs 1 Action). Qazalek uses a 1st-level spell slot to cast a spell that he has prepared.
  • Cast 2nd-level Spell (Costs 2 Actions). Qazalek uses a 2nd-level spell slot to cast a spell that he has prepared.
  • Cast 3rd-level Spell (Costs 3 Actions). Qazalek uses a 3rd-level spell slot to cast a spell that he has prepared.
  • Cast 4th-level Spell (Costs 4 Actions). Qazalek uses a 4th-level spell slot to cast a spell that he has prepared.
  • Cast 5th-level Spell (Costs 5 Actions). Qazalek uses a 5th-level spell slot to cast a spell that he has prepared.
