Staff of Unreality
The Staff of Unreality is an object that can supposedly make your wildest dreams come true, but could also awaken your worst nightmare, should the fates desire it.
The Staff has only ever been known to be used by one person, a gnome by the name of Rasyur Fiddlewanderer, who reportedly wanted to be as tall as his human friends. The Staff had no effect, so he sold it to a travelling merchant, and it has been lost to history ever since.
Mechanics & Inner Workings
The Staff of Unreality begins with 0 charges, and gains a charge every time it is used. Once the Staff has 20 charges, roll a d20. On a 16 or higher, the Staff loses all of its charges. On a 1-5, the staff explodes, and everyone withing 30 feet of it must make a DC 17 Dexterity saving throw, taking 20d4 force damage, or half as much on a success.
While attuned to the Staff of Unreality, you may make an effort to use it. To do so, use your action to say a sentence that you want to be true. Then, roll a d20. If the number rolled is between 20 and 20 minus the number of charges on the staff, then the sentence becomes true, as if you just cast the Wish spell. If the number rolled is between 1 and 1 plus the number of charges on the staff, then the sentence is warped, becoming the opposite of what you wanted to come true. If the d20 roll is between the two ranges, then nothing happens.

This is a singular item, which mainly exists in legends. One person has ever claimed to own it, though their actions with it, if any were successful, have not had much of an impact on the current world.
