Heart of Obsidian

Wondrous Item, Artifact (requires attunement)
Awakened and Exalted States. The Heart of Obsidian has an awakened and exalted state. The awakened state applies while you are just attuned to it, the exalted state applies when you replace your own heart with the Heart of Obsidian.
Awakened Properties of the heart

• The heart functions as a periapt of wound closure
• You gain a +2 to constitution up to max of 22
• While attuned to this artifact, you gain a +1 bonus to Armor Class
• You gain a hover speed of 20ft
• While attuned to this artifact your appearance slowly changes to look like Obsidian

Exalted Heart: The only way to turn the heart to the exalted state is to replace your own heart with the Heart of Obsidian.
Exalted Properties of the heart

• You are immune to poison, and necrotic damage
• Your alignment changes to Chaotic Evil
• The heart has 6 charges. You can use an action and expend 1 or more charges to cast one of the following spells (save DC 18) from it: Circle of Death (5 charges), Blight (4 charges), Bestow Curse (3 charges), Crown of Madness (2 charges). The heart regains 1d4+2 expended charges daily at dawn. Each time a spell is cast you must make a DC 18 wisdom save or become a puppet under Obsidian’s control
• You experience premonitions of danger and, unless you are incapacitated, you can’t be surprised
• You become vulnerable to radiant damage
• When you become attuned to the artifact, you gain a form of a long-term madness (You become attached to this artifact)
• You can do a 10 minute ritual that will tell you the location of anybody you wish (no matter if they are on another plane or world). The ritual can go into an exalted state which will bring everyone in a 150ft radius to the intended target. The only way to create this exalted ritual is to kill someone with the heart inside them with the Skofnung Sword. The ritual can last up to an hour.
