When your Vampire goes without satiating her/his compulsion for more than two days, roll 2d6+CON and get ready to hallucinate real and good.
On a 10+, you can no longer control your tongue and have to tell your companions of your need any chance you get until it's fulfilled. Also, ask the GM if there is anyone following you or wanting to hurt you in the 10 mile radius, and how close are they during your next short or long rest. Continue rolling after every long rest.
On a 7-9, ask the GM about anyone following or wanting to hurt you in a 10 miles radius, and be aware: your companions know that you have your cravings not just because you've told them as much, but because you talk with them through your hallucinations. You know they will only get worse and worse, unless you do something to stop the thirst, soon. Who knows what your visions of the "other" sides and worlds can bring, but at least you can sometime take note of your actual surroundings, when reminded by a loud word, or a bitch slap.
On a 6-, you may know who few of your companions are. You know what you crave. Everything else you (and your companions standing close to you) see and hear, the GM will tell you.
