Hellmouth
Millennia ago, the forces of the fiendish powers cut their way into our world. Using foul rites and demonic magics they constructed a permanent gate from their infernal realms into the material world. Through the quick work, bravery, and sacrifice of a few long forgotten heroes the initial invasion force was held back and a gate was constructed on our end of the portal to help stem the tide of evil.
The Hellmouth (Sometimes called the Hellgate of Fiendgate) is maintained, observed and defended at all times by an organization simply known as the Gatewatch. Gatewatch maintains forces not only at the Hellmouth itself, but also at Blackvine Fortress to the north, Greyweld Fortress to the west, and Flame Fortress to the east. These fortified locations defend all roads and passes into the mountains in order to not only keep fiendish incursions contained in the event of a breach, but also to keep interlopers and curious wanderers from stumbling upon the Hellmouth.
The gate stands in a large courtyard inside the central fortress, simply referred to as Hellmouth Fortress, the yard around it has a floor of solid stone slabs, as well as barricades facing the Hellmouth to offer cover in the event of a breach.
Along the sides of the Hellmouth there are numerous runes, but amid these runes are a line of ten spherically cut rubies, stretching from near the bottom of the gate to near the top, evenly spaced. These rubies act to gauge how close the gate is to being breached at any given time, as the rubies light up starting at the bottom, and as the nearness of fiendish forces grows, the rubies higher up the relic also begin to glow. This early warning system allows the fortresses to marshal their forces in preparation for an attack.

Manufacturing process
The Hellmouth was hand constructed by a team of Clerics, carved in sections from Marble and Granite, then blessed.
Each section of the gate was bound together with blessed iron fittings and silver runes of protection.

Significance
This powerful artifact is used to slow the tide of fiendish invasion from distant planes.
Without the construction of this gate, The Grand would likely have been lost to demonic invasion long ago.
