There have been cases of vessels becoming stranded in the space between spaces. Inevitably, the crew gradually goes mad. One one such vessel, the captain was first to succumb to the madness, lashing out against his crew. In an act of self defense, the first mate struck the captain in the head with a pipe - killing him and dislodging his eye in the process. The eye rolled away, but the first mate was quite certain he kept seeing it watching him.
Over time, the surviving crew began to think the first mate was going the way of the captain, and their suspicious treatment of him only compounded his paranoia - and due to the nature of the peculiar space they found themselves in, this fear and anxiety manifested itself physically.
It became an entity who would eventually go by Irwin Cornelius Upton. He does not like to talk about how he came to be the last thing left on that ship.
As a demonic nightmare entity, his anatomy is a bit baffling. Irwin is a big disembodied eye, roughly ten inches in diameter, with a mostly humanoid form made out of optic nerve and exposed muscle tissue. Maintaining the humanoid form requires conscious effort. In his sleep, he appears to be a pile of flesh with a large eye resting atop it. He can squeeze through any opening larger than his eyeball.
