Symbolism or Spell
A wizard uses a magic word over an item, or simply gives it a particular meaning to turn it into a link to other person.

Effect
The caster and the target can "hear" each other's thoughts.
Side/Secondary Effects
Some think that certain strong emotions can be shared trough the link. Others fear that those involved lose track of who was the original owner of each thought.
Physical Restrictions
It requires the target's acceptance. (Or a very strong will or subterfuge on the caster's side, but that's very rare). For these reason, it often requires that they are communicating somehow.
