"Lords and Ladies" range from influencial significant families to minor Lords and Ladies of smaller families of lesser means.
Of course, even a "lesser" families have more influence and resources than the majority of the low born.
Mostly a title appointed by a ruler at some time and thence inherited thereafter, unless a family falls into dishonour, disrrepute or is wiped out violently.
Houses generally lead by minor lords or ladies (some eschew the title to be clear) are often the backbone of factional armies.
