transition zones

Transition Zones are regions where two planes are in close contact with each other and begin to blend together.  The existence of such zones has been a long-accepted fact going back for millennia; the elemental planes are all connected to each other through transition zones, and the Prime Material Plane has always had a few small ones connecting itself to the Feywild or Shadowfell.
