You attempt to search the surface of target humanoid's thoughts. This humanoid must succeed on a Intelligence (?) Saving Throw, having their mind read on a fail and repelling the spell's advances on a success. If the humanoid is unconscious, incapacitated, or asleep, they automatically fail this save. If the caster is currently seen by the target and/or if the caster is currently in combat with the target, the target gets Advantage on the save.
On a failed save, you glimpse into the mind of the target humanoid, gaining insight into their desires and motives, the current and most immediate goal (desire, current objective, etc) that the creature has, and the names of locations of people associated with them.
If this was cast in combat, you glimpse their intent to move before they even move a muscle. You have Advantage on Attacks and Spell Attacks against the target creature until the end of your next turn. Additionally, Attacks and Spell Attacks made by the target creature have Disadvantage if they target you.
If this was cast outside of combat, you learn of what the humanoids current tasks or intentions are. If they are part of a broader web of intentions, you gain knowledge that there are broader implications, but do not learn the specifics. In terms that do not reveal the full identity of other creatures involved or the creature this spell was cast on, you learn the task that they are doing or going to do and the reason why they are doing it. If other creatures are involved in this, you learn of them, but only by their faces and the names the targeted creature knows them by.
If this was cast on a creature that is incapacitated, unconscious, or asleep, you learn of the plans that this creature has been apart of in the past week (that that creature has knowledge of) including at the moment of casting. You learn full names of the creatures involved with their plans and duties (as full as the targeted creature knows them by.
