Burning Lung

All tobacco grown in the City of Brass is infused with part of the elemental plane of fire it develops in. Smoking this tobacco, called Dolkha, can have mystical effects on the user causing it to be in great demand throughout the planes. What is less know is the detrimental effects it can have on one's body.
Aside from the common problems with smoking tobacco (mouth, throat, and lung issues) Dolkha can cause the user to crave the heat of the elemental Plane of Fire.

A character must make a Poison saving throw with advantage at the end of every day that Dolkha is smoked or contract Burning Lung. Characters with Burning Lung have trouble focusing in weather under 98° Fahrenheit rolling all intelligence and wisdom saving throws that are not active with disadvantage.

One who contracts Burning Lung have a shivering chill when the temperature is below 98° Fahrenheit, visually shaking; becoming cold and clammy to touch when standing still. For some reason motion negates these effects, either by distracting the sufferer or raising their core temperature. Once contracted, Burning Lung can only be cured by a greater restoration spell or more powerful magic.

Burning Lung can only be contracted from smoking Dolkha.

Prognosis

Burning Lung comes on randomly, one can live a full life without being cured as long as the dwell in a hot place.
