Reincarnate

You touch an intact dead humanoid, if the creature is lacking body parts or organs integral for its survival – its head, for instance – the spell automatically fails. Provided that the creature has been dead no longer than 1 minute since beginning to cast this spell, the spell then replaces that creatures soul with the soul of another dead creature of your choice. Provided that soul has been dead no longer than 10 days and is free and willing to do so. The creature then returns to life with the new soul and 1 hit point.
A soul that has been dead longer than 10 days can still be reincarnated using this spell, so long as it has continued existing in another form such as being under the affect of the Raise Dead spell.
The reincarnated creature recalls its former life and experiences. It retains the capabilities it had in its original form, except if applicable it exchanges its original race for the new one and changes its racial traits accordingly.
Coming back from the dead in this manner is an ordeal. The target takes a -4 penalty to all attack rolls, saving throws, and ability checks. Every time the target finishes a long rest, the penalty is reduced by 1 until it disappears.
