Resurrection Flaws
With resurrecting physical bodies and souls, certain spells and rituals may bring about unwanted changes. Unfortunately, not much can be done about flawed resurrections in terms of deterioration of the soul. But there are certain druidic rituals and spells that can reincarnate the soul if the body is lost or to repair a dead body that cannot handle a soul. Regrettably, those forms may be of different ancestries or cultures that the soul may reject. Thus it is possible for a body to falter that has been resurrected by a reincarnation ritual if the soul rejects it, however the timeline for this rejection varies.
Thus there is a ritual spell that could revert the physical form back into the soul's memory of what it looked like. But the ritual takes some time to prepare and some reagents of a rare sorts.
Components Needed:
- Blood or Bone Dust of the Ancestry You Were Born Of.
- A memory of which you showed your ancestry's special skills.
- A black pearl worth 200gp.
The spell can be performed by any spellcaster but only one of advanced spellcasting ability. Those who know at least 4th level advanced magic.

To Bring back what once was mine
With this offering of my people's race
Fix that what was thought to replace
