Magical items are not really permanant, ever
Magical items in Turian are never permanent, not really. Regular magical items have a certain number of uses, sometimes different uses, stored in them and may or may not be reloaded.
Permanent items have an unlimited number of uses installed, they basically recharge from the ambient magic. The speed of reloading makes the number of uses in a certain timespan. However, both kinds of items, in fact, all magical items may suffer from unexplainable failures. This is a known fact.
For a regular item, you should roll two dice once per reload. On a specific roll on both dice (chose a type of die and a number or range of numbers) the item does not work and must be reloaded again. If a third die shows the same number (or range) the item loses its enchantment. For a permanent item apply a similar rule but always have at least two random numbers (more if you want to).
The third roll can be considered optional or you might decide on a save for the character. Personally, I often do the latter. Sometimes I employ a loyalty value for items which work as a regular skill check to see if an item might just not work for this one hero any more, but might for another.
A Magical Artifact is NOT a magical item per se. It is something more rare, wonderous and fantastic. They are often the work of demigods, gods, daemons or deamons etc. they are not supjetcted to the suggestions above.
