At +4 you treat five patients during a single day, each requiring their own check.
In combat, your will to help people spurs you to action. If you intend to make a First Aid (or equivalent) check in combat, you act first before any other actions take place. You reach your patient and start treating them, then the rest of the actions happen. In addition, you may ignore debilitation due to wounds when performing mid-combat treatments, up to the value of this trait. At +4 you can ignore all your debilitation, regardless of how high.
