It's a volcanic island, the blown caldera on the north side providing shade and refuge for the opening to the sloped, curved underground city. The south side of the island is magma and lava flow, which occasionally leaks down to the city below. Over the years, the city has worked around the lava flows, making them into rivers and building around them. The people of Fellspire have adapted to the extreme conditions, and many wear clothing woven with cooling enchantments. The cryogem mines, Fellspire's primary export, provide employment for a significant portion of the population. The cryogems are not just economic assets; they are also revered for their role in keeping the city habitable. Festivals are held to celebrate the gems and the cooling magic they bring.

The surface island of Fellspire is split in half - surrounded on one half by an active volcano that flows downward to the ocean, and the other half of the island that contains the archway into the depths. The city has winding lava flows, causing intense heat and occasional rivers of lava. The city is tiered, with upper levels closer to the surface and lower levels descending into the heart of the cavern. The mines break off into the rock. At the bottom of the slope of city is a pool of lava.
