Carnabrex mutagen

  This mysterious bubbling green fluid was created by the Carnabrex Coterie. If imbibed by a non-construct creature, that creature must succeed on a DC 16 Constitution saving throw or permanently gain a random effect from the table below. These effects can be removed by five points of lay on hands, remove curse, greater restoration, or stronger recovery magic.
  A creature can only have one such mutation at a time; if it would gain another one, it instead takes 10 (3d6) poison damage.

1 Your eyelids are closed shut. You are blinded, and have blindsight out to 30 ft.
2 Your skin is cold and clammy to the touch. You are resistant to cold damage and vulnerable to fire damage.

4 Your ears are overrun with earwax. You have disadvantage on Perception checks that rely on hearing. You can pluck out a glob of earwax, which can be consumed to restore 1 hit point. A character can only benefit from this effect once per long rest.
5 You develop a bestial awareness, and have advantage on Perception checks that rely on sight and hearing. Animals are threatened by you, and consider you hostile and predatory. You have disadvantage on Animal Handling checks.
6 Your mouth seals shut. You no longer need to eat or drink and gain the ability to communicate telepathically with any creature within 30 ft with whom you share a language, but otherwise cannot speak.
7 Your back becomes hunched. You have advantage on Intimidation checks but disadvantage on Acrobatics checks and Dexterity saving throws.

9 Itchy, cankerous boils and cysts erupt over a part of your body. They are a constant distraction, imposing disadvantage on Perception and Investigation checks. The fluid in the cysts grants resistance to acid damage.
