Fumbles - Spellcasting - Natural 1 on Spell Attack Rolls
When a spell requires an attack roll (melee or ranged) and the roll is a Natural 1 not only does the attack fail but the Spellcaster must make a saving throw based on the casting ability used (e.g. Wizards would make an INT Save). The DC is 10+ the level at which the spell was cast*. Failure results in having to roll on the Player's Handbook Wild Magic Table or the DonJon Wild Magic Surge tables and the Spell Slot being spent.
*Example: Acid Arrow is a 2nd Level spell requiring a Ranged Spell Attack roll. If cast at 2nd Level the DC would be 12. However, if cast using a 5th level slot, the DC would be 15.
This of course is meant to reflect the complete miscasting of the spell. Perhaps the gesture was wrong, or not quite enough of a material component was used or it was spoiled. Another possible explanation would be that the caster mispronounced the verbal component. Any of these could be due to the distraction of combat.
