Captain of the Military
Captain is the highest rank that an officer can receive and can only achieve it through years of service, or through elite training that only the wealthy and nobility can attend.
The Captains will lead entire armies, or specialized units with vital missions.
Qualifications
One must have a long history of exemplary service, or must be born of a wealthy or noble family and able to afford the elite training.
Appointment
When a person meets the qualification to receive the rank of Captain, they are brought before the King, who will choose whether or not to grant the people the rank.
Duties
Captains are the forefront of the military, holding all responsibility and honor that comes with their position and actions.

Grounds for Removal/Dismissal
If a Captain brings too much shame and dishonor to their name and rank, they are removed and possibly executed.
History
The rank of Captain was honorary at first, being granted to the first born sons of nobles, but as war broke out between the Humans and the Orcs, the rank required more than just prestige, but also skill. After many nobles lost their prized first bloods, they demanded that the King ensure their son's safety by providing them with proper training. Elite training was developed; but those too lazy to properly put in the effort would often slip threw the cracks by bribing their teachers or simply refusing to do the work.
To ensure the success of the training, the King ordered that all that went through the training were tested and those that past would have to present themselves infront of the King for final approval. Eventually those of wealthy families were allowed to have their children train as well, and as the war increased in threat and danger, other children beyond the first born were allowed to train as well.
It wasn't until a skilled lieutenant showed great skill and honor on the battlefield that someone not trained by the elite was allowed to be granted the rank of Captain.

Captains are the face of the military and their actions reflect directly onto the King himself.
