Soul Damage

Soul Damage is what happens when things go wrong when you try to mess with magical forces.
It represents harm done to your very soul and is permanent.  It can range from trivial things like your hair changing color to more severe fates like all your limbs falling off.  It can even involve your own death.
The severity of the damage is usually proportionate to whatever it was you were tying to do.  The more powerful the effect you hoped for, the worse things can get.  
Like all things in magic, it is impossible to fully predict.
