Makub's Revenge

"Your bowels turn to liquid fire, your skin heats up hot enough to steam in the rain, and solid food becomes a distant memory. If you caught it from the water, there's a good chance your throat'll try to close up. If you got it off a goblin shank, the wound will turn black and start smelling like a whore in a fish market. If you die, your grave might spawn goobers, and if you live you'll wish you hadn't for a while. The real lesson here is that if you go to the Badlands, don't drink the water and don't get stabbed."   ~Margrave Arronax Skinner

Transmission & Vectors

Consuming food and water contaminated with greenskins blood or offal. Piercing or slashing injuries with nork weapons.   At the point of infection, the character must roll a DC 12 Constitution save. On a failure, they have contracted the disease. In 1d4 hours, they acquire the Poisoned condition and can no longer heal naturally.

Symptoms

The first sign of Makub's Revenge is a high fever accompanied by muscle weakness. Soon after, bloody diarrhea and vomiting. Swelling and necrosis around the infection site mark the final stages of the disease, where the victim begins either recovering or dying.   Every 8 hours after the onset of infection the sufferer must make a DC 15 Constitution save. On a failure, the character receives 1 level of exhaustion. On a critical failure they receive 2 levels of exhaustion. They cannot regain these levels by resting until they are cured. After 3 successes in a row, one success more than 5 points above the DC, or one critical success, the victim fully recovers and can begin healing normally.   A DC 15 Constitution save is required every meal to hold enough food down to benefit. A failed save has a 5% chance of creating 1d4 goobers   If the victim dies, there is a 15% chance that the grave site becomes a spawning ground for goobers, with 2d12 of them fruiting out and breaking loose.

Treatment

A Cure Disease spell or potion works to instantly cure the condition, though the damage already done remains to heal normally.   A Healer's Kit can be used to treat the illness with a DC 15 check, giving the victim advantage to each check while being treated.   Getting blind drunk has the same effect as a Healer's Kit check.

Prognosis

The disease will kill the weak, the infirm, and the very young every time. Strong adults in their prime have a good chance of eventual recovery, though the memories will remain with them for a very long time.

Hosts & Carriers

Goblins are immune to Makub's Revenge. Elves have disadvantage to their saves against contracting the illness.

Prevention

Avoiding contact with water or sharp weapons in Uzgilbat is the best way to avoid contracting the illness.   Potions and spells that prevent disease can help, as can treating the water with alcohol or vinegar before drinking it.

Cultural Reception

Due to the possible presence of goobers around the victim, they tend to get a wide berth from anyone not directly attempting to help them.
