King and Queen
Between the noble houses, one house rise above the others and claim the throne for his and his ancestors. Such has it been for ages, such will it be for a long time. Sometimes the rise happens through military might and deals of allegiance between the houses, or by diplomatic deals with the threat of death and slaughter by a, sometimes, fictional army.
If by chance the person on the throne is male, his title will be King, in the case of a female, her title will be Queen. Other ways to gain those titles are by marrying into the throne. Such as a male king takes a female queen. While a female queen takes a prince. In the case of a king taking a prince or a queen taking a princess as their spouses, then the titles will change accordingly.
Should the sitting king or queen fall then the nearest successor rises up. A queen remains a queen, a prince gain the title of king, princess gains the title of queen. Should the pair produce any heiress then their titles will that of Crown prince/princess. Followed by 2nd prince/princess and followed by 3rd, 4th, etc.
In the case of not being any nearby successor then it falls to the noble houses of finding the house that will follow the king and queen. That comes back to which house can successfully rise up. Be that either by military might or be diplomatic scheming.
