Helm of the Red Dragon
The Helm of the Red Dragon is one of the six pieces of the Armor of Tiamat, created from the scales of the first red dragon.
It was worn by Azbara Jos during the Battle at the Well of Dragons and was in the Chromatic Pentagram when it returned to Avernus.
Mechanics & Inner Workings
On its own, the Helm of the Red Dragon appears to be an ornate helmet made of red dragon scales fit for a large creature. However, when a creature touches the helm, it shrinks or grows to fit their size. When worn, the helm melts into the wearer's head, burning away the original skin and replacing it with that of a red dragon. The helm can then not be taken off short of magical means.
With time and proper attunement, the Helm of the Red Dragon can grant its wearers many abilities. The helm can secrete a magical flame that can allow its wearer to breathe fire. A properly trained user can also revert the dragon head back into a helm. The cloak can also allow its wearer to transform into a red dragon of increasing age, depending on the wearer's level of attunement with the item.

Significance
The Cloak of the White Dragon was created from the first red dragon as a way to preserve their power even beyond death. As a piece of the Armor of Tiamat, the gauntlet resonated with Tiamat and could assist in her summoning, as attempted by Don Rel during the Great Dragon War.
